1) I'm testing a VPN right now, and accessed IP/DNS Detect to test ipv4/ipv6/dns leak, and everything is ok, but it has this message: "Fallback:fail". The explanation says:

Tale a look. The site shows a Ipv4 and Ipv6 ip, as the DNS adress from the VPN service, but it has that fail on fallback

2) Besides that, another VPN i've tested, shows that ipv6 test not reachable. What this exactly mean?

IPv6 has been know to leak your IP is some VPNs , if it's not leaking or showing, that's a good thing.
Besides that, another VPN i've tested, shows that ipv6 test not reachable. What this exactly mean?
It means that that particular vpn does not support ipv6..nothing to worry about..
