Do you have the real-time modules activated in MBAM Premium, or disabled?

MBAE isn't an on-demand scanner and only works in real-time, you can add it to Real-Time Protection instead. (How-to)

Zemana set on RT is not needed when you have EAM + WSA, better let it OD.

Hello @Cerberus, you've quite an impressive configuration. I don't think I'd completely missed seeing it when first posted, ..but I was probably (then) struggling to keep my eyes open!
I'd particularly liked your practice of two-step authorization, and your exemplary scores on LastPass' challenge gives me something to shoot for too!

To set Chrome's built-in blocker (for third party cookies).

1. Choose settings, and scroll down to "Show advanced settings".
2. Under "Privacy" <click> the "Content Settings" button.
3. Check the box beside "Block third-party cookies and site data".

• Additionally, you may choose to mark the circle "Keep site data only until end of session"

I've got rid of ESET Online Scanner and replaced it with Zemana AntiMalware. Real Time Protection is set to OFF. I've also added AOMEI Backupper.
