It doesn't. Emails containing malicious items will be blocked but no Anti-Spam support for Thunderbird/Windows 10 Mail app:

* ESET products will scan POP3/POP3S and IMAP/IMAPS email for the presence of malicious code while using Mozilla Thunderbird 6+ or Windows 10 Mail or other email clients not listed above. Email may not be scanned for spam, however.

I think you have to check Options ---> Security ---> Antivirus
Then tick to allow antivirus to check your email.

I hope this will suffice.

ESET is able to check emails both through https protocol filtering and with a dedicated plugin. The plugin only works with Outlook but https protocol filtering does not depend on email client. As others reported, anti-spam filtering only works with the plugin though.
