Pop-up Survey in Yahoo removal

Hello, I've tried multiple ways to find and get rid of a pop-up survey that appears in my Yahoo mail. I've been unsuccessful. Please help.

Is this problem occurs only when using Yahoo mail?

Yes. I am a Virtual Assistant and I work in both gmail and outlook all day and this pop-up only appears in my personal yahoo account.
